Speech Language Pathologist (SLP) – Home Health | Post-Acute Care

Job Description:
We are seeking a dedicated Speech Language Pathologist (SLP) to provide patient-centered care in a home health setting. The SLP will evaluate, diagnose, and treat patients with speech, language, cognitive, and swallowing disorders in a post-acute environment. This role requires strong clinical judgment and the ability to work independently while delivering care in patients’ homes.

Key Responsibilities:

• Conduct speech, language, cognitive, and swallowing evaluations
• Develop and implement individualized treatment plans
• Provide therapy to patients in a home health setting
• Monitor patient progress and adjust treatment as needed
• Educate patients and caregivers on therapy plans and techniques
• Maintain accurate and timely documentation
• Collaborate with interdisciplinary care teams
