Job Responsibilities:
Assessment: Performs assessments and reassessments per policy or as warranted by patient condition.

Development and Implementation: Initiates, prioritizes, and implements the plan of care.
Reviews and revises the plan as indicated.
Collaborates with patient/family and interdisciplinary services.
