Case Manager II – Family Stabilization

Volunteers of America Mid-States is seeking a Case Manager II to support individuals and families experiencing homelessness as they work toward stable housing and greater independence.

What You’ll Do

• Provide assessments, case planning, follow-up, and ongoing support.
• Connect clients with housing, healthcare, mental health, substance use, and community resources.
• Advocate for clients and help them access benefits and essential services.
• Build relationships with landlords, community partners, and service providers.
• Facilitate the Critical Time Intervention (CTI) model.
• Maintain accurate case notes, HMIS, and required documentation.
• Identify barriers to housing stability and help develop solutions.
• Provide culturally responsive, person-centered, and harm-reduction services.

What You’ll Need

• Master’s degree in Social Work or related field with fewer than 5 years of experience; or
• Bachelor’s degree in Social Work or related field with 5+ years of related experience; or
• 9+ years of related experience without a degree.
• Strong communication, organization, advocacy, and problem-solving skills.
• Valid driver’s license, reliable vehicle, and required insurance.
